Your first visit will allow us to evaluate your symptoms.  As part of the initial appointment, we may request additional tests or procedures, such as blood tests, imaging studies, or endoscopic examinations for diagnosis or treatment.  A treatment plan may also be prescribed along with a follow-up plan of care. 

When you come for your appointment please remember to obtain and bring the following:

• Insurance card
• Driver’s License or a valid ID
• Referral Letter (if required)
• Copayment (if applicable)
• List of medications (Prescription and over the counter)

To save time, you may print and complete new-patient forms from the Patient Forms section, and bring with you at your appointment.
